Shashi Kapur, D.D.S., M.Sc.D., B.D.S.
Dr. Kapur received his B.D.S. from Punjab University in Amritsar, India in 1969.  After working in Zambia, Africa with the Flying Doctors of the United Nations, Dr. Kapur emigrated to the United States in 1973.  He completed his pediatric residency at Boston University in 1975.  He turned to teaching and taught at the dental programs at the University of Southern California and Baylor in Texas.  After helping in the training of dozens of residents throughout the years, Dr. Kapur was interested in applying his knowledge to direct patient care.  He attained a second dental degree from the University of Southern California in 1987.  Armed with his advanced dental training and diverse experiences, Dr. Kapur opened up his first office in Chandler in 1989.  He moved his office to a brand new complex across from Chandler Regional Hospital in 2002 and opened his second office in West Phoenix in 2006.  Dr. Kapurís interests include reading, travel, cooking, and spending time with his family.  He is also an avid fan of all things canine related!
Mukul Dave, D.M.D.
Dr. Dave joined AZPD in summer of 2012. Not a stranger to moving he has lived in IL, OR, MN, MA, NC and PA prior to making AZ his home. Dr. Dave attained his dental education at Boston University School of Dental Medicine. He joined the Navy following dental school and served as a Naval officer and general dentist on Marine Corp Base Camp Lejeune NC. His desire to specialize in Pediatric dentistry brought him to Childrenís Hospital of Pittsburgh where he realized his true passion for the specialty. Away from adventures of workspace, he enjoys painting, spending time with family, exercising and traveling.
Henry Martinez, D.M.D.
Dr. Henry Martinez was born and raised here in Arizona. He grew up in the East Valley and attended Dobson High School. Dr. Martinez obtained his Bachelor of Science degree in Kinesiology at Arizona State University. He then went on to complete a Masterís degree in Biomedical Sciences at the Rutgers Graduate School of Biomedical Sciences in Newark, N.J. (formerly the University of Medicine & Dentistry of New Jersey). He obtained his D.M.D. (Doctor of Dental Medicine) at the Arizona School of Dentistry and Oral Health in Mesa.

Following dental school, Dr. Martinez followed his passion for providing dental care to children and people with special needs by completing his pediatric dental residency training at the Bronx Lebanon Hospital Center in New York City. There he received extensive training in all aspects of pediatric dentistry including sedation and hospital dentistry. He has received extensive training in treating people with special needs during dental school at the Arizona School of Dentistry & Oral Healthís Special Care Unit and as a pediatric dental resident at the Rose F. Kennedy Center at Jacobi Medical Center in New York. During his spare time Dr. Martinez enjoys spending time with his wife Irina, who is also a dentist, and family. He enjoys working out, watching movies, and golf.

David P. Ember, D.D.S.
Dr. David Ember is a graduate of the Ohio State University College of Dentistry and a former resident at Nationwide Childrenís Hospital in Columbus, Ohio. He has practiced in multiple locations including almost twelve years working for Aramco in Saudi Arabia. He currently fulfills his global wanderlust through his charitable endeavors overseas as a clinician and a director on the board of Childrenís Dental Care International. This organization has set up dental clinics in Sri Lanka, Ecuador, and Brazil.

Erin Hughes-Carey, D.D.S.
Dr. Hughes-Carey, a native of Altadena, CA, received her dental degree at Meharry Medical College in Nashville, TN in 2005.  She completed an Advanced Education in General Dentistry at the University of New Mexico in 2006.  After completing her AEGD, Dr. Hughes-Carey worked two years in San Luis, AZ as a general dentist for the National Health Service Corp.  She then continued her training and completed her Pediatric Residency from UNLV in 2010.  Dr. Hughes-Carey is an active member of the American Academy of Pediatric Dentistry and the National Dental Association.  She enjoys spending time with family and friends.  Her interests include sports, dancing, Special Olympics, and church activities.


Rick Meyers, D.D.S.
Dr Rick Meyers grew up in a suburb of Pittsburgh, Pennsylvania. He is naturally a Pittsburgh Steeler fan. Dr Rick graduated Magna Cum Laude from the University of Pittsburgh. He attended dental school at the University of Texas Health Science Center at San Antonio. After receiving his doctorate, and driven by his love for children, he continued his post-doctoral education and specialized in pediatric dentistry. He is proud to call his wife Holly his best friend (26 years and counting). They have three awesome children Katie, Corey, and Cameron. Dr. Rick also has more dogs than he would like to admit. He really enjoys any kind of sport whether it's watching, coaching or playing.


Jean Lewis, D.D.S.
Everyday I wake up thankful to work with children in a place that feels like home. I feel very blessed to have the opportunity to work with a team of people who are focused on doing their personal best to provide the best possible care. For me, it is a privilege and an honor to be a pediatric dentist.
I grew up in a small town just north of Pittsburgh, Pennsylvania. I am the youngest of seven and have ten nieces and nephews. Seeking blue skies and sunshine, I settled in Arizona in 2008. I completed my dental education at the University of Pittsburgh School of Dental Medicine and my pediatric dental residency at Rutgers University, formerly the University of Medicine and Dentistry of New Jersey.
Professionally, I am active in the state and local dental society. I am a member of the committee for dental education for the Arizona Dental Association, a member of the American Dental Association, and a diplomate of the American Board of Pediatric Dentistry. I feel that lifelong learning is an obligation and my professional continuing education goes above and beyond the minimum required for licensure.
Giving back is also a way of life for me. I volunteer at local soup kitchens, the Arizona Mission of Mercy and with other dental organizations throughout the Valley.
On the weekends, I can be found hiking South Mountain, road biking, rowing at the Tempe Town Lake or taking in a movie. If I have a long weekend, I generally hop on a plane to see my family and to eat as many home cooked meals as possible. I love the Pittsburgh Steelers, Penquins and Pirates. However, I have been known to root for the Cubs in a pinch. Thank you for taking the time to get to know me. I hope to build a long-lasting relationship with your child.
